The frequency of a sound wave changes depending on the direction of movement. When red blood cells move toward the probe transducer, the sound frequency increases; when they move away, the frequency decreases. This frequency shift helps estimate the speed and direction of blood flow, a phenomenon known as the Doppler effect.

A small transducer that sends and receives sound waves is placed on the chest. It emits sound waves and records their echoes, creating images and flow maps on the monitor.

A Doppler ultrasound helps to diagnose conditions such as:-

  • Heart valve defects
  • Blood clots
  • Blocked arteries
  • Decrease blood circulation in the legs
  • Carotid artery stenosis

Types of Doppler echocardiograms

The different types of ultrasounds are:-

  • Color Doppler: Changes in the sound waves are represented in different colors to show the direction of blood flow.

Setting a high color velocity scale shows the absence of flow. A low color velocity scale shows fast flow, which looks chaotic or reversed.

What do colors represent in Doppler echocardiography?

It uses colors to assess the flow of blood. The colors help to distinguish between the direction and speed of the blood flow in vessels. The most common colors are:-

  • The red color indicates blood flow towards the transducer, showing arterial blood flow, which is oxygen-rich and flows in pulsations.
  • Blue color indicates blood flowing away from the transducer, representing the venous blood flow, which is slow and less pulsatory.
  • Green color indicates low-velocity blood flow, as seen in capillaries, and is utilized in advanced color Doppler ultrasound systems.
  • The colors yellow or orange represent turbulent blood flow, which is seen in stenosis or other abnormalities in blood vessels. The mixing of colors is due to blood flow passing through a narrow opening.

Besides these colors, the ultrasound may use other colors or shades to represent flow velocities. The specific colors used depend on the ultrasound system and the physician's or technician's preference.

Role of colors in color Doppler Echocardiogram:-

The direction of blood flow to the ultrasound transducer is indicated by the colors red and blue. Red color represents blood flow toward the transducer, and blue represents flow away from the transducer. It helps us identify patterns that indicate abnormalities, allowing us to assess the severity of the disease.

  • Direction of blood flow

Red indicates the direction toward the transducer, and blue indicates the direction away from the transducer, helping us evaluate the direction of blood flow. It shows us abnormal heart circulation.

  • Velocity

The brightness of the color indicates its velocity; brighter colors show high velocity and high turbulent flow areas.

  • Abnormal flow patterns

We can detect regurgitation or backflow through a valve, as well as shunts or abnormal connections between the heart chambers. A red jet flow in the opposite direction through a valve into the chamber indicates a leak (regurgitation).

  • Quantification

It helps us to identify the extent and severity of regurgitation (leak) or shunt by observing the color flow patterns.

  • The turbulent flow 

It is chaotic and irregular. It occurs when blood is forced through a narrow area or when abnormalities occur in the vessel.

In the color Doppler echocardiogram, yellow or orange colors indicate areas of concern, while green indicates normal areas.

It is because they indicate turbulence in blood flow, which is caused by blockage or other abnormalities in blood vessels. The colors, when they get mixed, appear in unusual patterns, indicating issues in the heart.
